Nurses Corner

Communication is the cornerstone to all good relationships. I support regular contact with parents and look forward to speaking with you about your child. It is important to make the nurse aware of any significant changes in your child’s health status, including any serious illness, injury or surgeries that your child experiences during the school year. Please feel free to contact me with questions or concerns pertaining to your child’s health and wellness at any time. It is equally important that the school be able to reach you or a designated responsible adult in case of an emergency. Throughout the year, please update your child’s emergency contact information card. When there is a new job, new home, we need to have the updated information for your child’s records.  Please note that a new card will need to be completed in the beginning of each school year.  This is a state requirement and is important for your child's safety.

Sports Physicals must be completed annually if your child participates in any school sports. A complete Delaware Interscholastic Athletic Association (DIAA) form must be completed after April 1 to be eligible to play a sport in the following school year. Please send your child’s completed physical to the school nurse to ensure that it is placed in his/her health record. A list of students who have a completed DIAA physical forms will be available to all coaching staff. The DIAA sports physical form should be used when the physical is completed. The DIAA form is a five page form that must have all areas completed before your child will be eligible to participate. Incomplete forms will be returned to the student. The DIAA form is available in the main office of the school, the school nurse's office, the wellness center or online under links on my page.

Please make the necessary appointments to get your DIAA physical completed and turned in to Milford High School, Milford Central Academy, or the school nurse's office. Any physical completed on or after April 1, 2023 will cover you for all of the 2023-2024 sport seasons! For student's with asthma, inhalers, epipens or medical conditions, please see the school nurse to ensure that all paperwork is completed including asthma action plans, medication consents, permission to self carry, etc.

DIAA Physical Form

1. Unless directed by your child’s doctor, medication that is given once, twice or three times a day should be given at home.
2. Medications must be sent to school with a responsible person, preferably a parent.
3. All medications must come in their original container. Prescription bottles must have the most recent prescription         label.
4. All medications will be given according to the prescription or manufacturer's recommendations.
5. Written consent to give any medication is required. If for some reason a parent is unable to bring the medication to school, the parent must send in a written medication consent. This consent should include the full name of the child, the reason that the medication is being given, and when the child received the last dose of the medications.
6. Children are not permitted to carry any medications during school. There are special circumstances when students with diabetes, asthma, and life threatening food allergies may carry certain medications that are considered life-saving treatments. If your child needs special accommodations for a medical condition, please contact the nurse’s office.

A parent is the best judge of whether a child is well or sick. School nurses use guidelines to assist parents in making decisions when it pertains to allowing a child to remain in the educational environment.

You will be requested to pick up your child from school if he/she has a fever of 100.0 F or higher, or has symptoms of illness that interfere with his/her ability to actively participate in their education or pose a health risk to other children.

If your child has been sick during the weekend or overnight, please know that your child must be symptom free for 24 hours before returning to school (i.e. fever, vomiting, diarrhea etc.).

Please do not send your child to school with instructions for the school nurse to determine if they are sick or injured. As a parent, you are the best judge of your own child’s wellness and medical needs.
