The State of Delaware requires students who are new to the Delaware Public School to provide basic medical information prior to entering school for the first time. It is critical that theses documents be provided to the school nurse in a timely fashion. If these documents are not provided, your student may be denied entrance to school or excluded from classes until they have been provided.
The following are the State of Delaware requirements:
(1) Physical exam completed within the past 24 calendar months on entry and prior to entry into 9th grade
(2) Tuberculosis skin test completed within the past 12 calendar months or completed Risk Assessment Questionnaire

*DTaP (5 vaccines in series)
Tdap (booster, prior to 9th grade entry)
Meningococcal (prior to 9th grade entry)
*OPV/IPV (4 vaccines in series)
*MMR (2 vaccines in series)***
*Hepatitis B vaccine (3 vaccines in series)
*Varicella (2 in series) or physician documented history of past chicken pox
*Lead BLOOD test- All Kindergarteners (lead screenings are not acceptable)
***These items must be completed before the student starts school for the first day.
