Milford High School and Milford Central Academy Athletics will be transitioning to Rank One, a new online platform for all athletic registration (https://www.rankone.com) and required DIAA Physical forms for the 2026–2027 school year.

After May 4: All physicals and registrations must be submitted online through Rank One (https://www.rankone.com) Registration will open after May 4.

• A new DIAA physical (PPE) form is required for all student-athletes
• All eligibility forms (concussion, cardiac arrest, heat policy, etc.) will be completed electronically through Rank One (https://www.rankone.com)
• Paper submissions will no longer be accepted moving forward

• Create a Rank One parent account
• Add their student(s) using name and date of birth
• Complete and sign all required forms online
• Upload Page 3 of the student’s physical (medical card)
