Bayhealth Wellness Center: Meeting the Needs of Today's Adolescents
Our Background:
The Wellness Center is operated by a partnership that includes Bayhealth Medical Center, Milford School District, and the Delaware Division of Public Health, Department of Health and Social Services.
Located in the high school, the Wellness Center helps teens overcome many obstacles to receiving good health care - obstacles such as lack of transportation, inconvenient appointment times, or worries about cost and confidentiality.
There is no charge for students to be seen in the wellness center, however we do bill insurance companies. Students or parents will not be expected to pay traditional co-pays or fees for any services.
The center is structured to meet the physical needs of today’s adolescent as well as the nutritional, mental health, and health education needs. The focus of the Wellness Center is on preventative services with a goal of promoting positive physical and mental health.
Our Mission:
The mission of the Milford High School Wellness Center is to provide our students with a means of obtaining health services that can be coordinated with each teen’s family physician.
Adolescents are typically a medically under-served part of the population. Access to health services on site and education regarding healthy lifestyles should increase school attendance and improve the student’s ability to concentrate in class.
Accessing Services:
All Milford High School and eighth grade Milford Academy students may enroll in the Wellness Center. In order for the student to receive care at the Wellness Center, parents/guardians will need to complete a registration packet. This includes a new consent for services and health history form. These forms must be complete and returned to the Wellness Center before the student may be seen. (**See below for down-loadable forms)
Students may be referred to the Wellness Center by the school nurse, guidance counselor, or school administration. Parents or students can refer for services as well. For students to be seen by a provider in the Wellness Center, they must be a member.
Hours of Operation:
The Wellness Center is located in the back of the high school in Room 319. It is open from 7:00 am – 3:00 pm on days school is in session. Limited hours are available during the summer.

***Please download, print, and sign the forms below for wellness center membership/ sports physical. A student must be a member of the Wellness Center and have a completed DIAA Form before they can be seen in the wellness center for a sports physical exam.
